2025 QIP Collaborative: Sustaining Momentum: Leadership Strategies for Continuous Improvements

The 2025 QIP Collaborative comes at a critical time as Ohio Medicaid reimbursement undergoes significant changes. These shifts will have a direct impact on nursing home providers, and this collaborative is designed to ensure that participants are fully informed and prepared to navigate these changes. Throughout the series, providers will receive timely updates on these developments, helping them understand how the evolving landscape will affect both their quality measures and reimbursement. Additionally, LeadingAge Ohio is excited to share that participants will meet the new Ohio Administrative Code language for the Nursing Home Quality Initiative found in Chapter 173-60 of the OAC, which became effective in November, by participating in this collaborative. This collaborative will build on the evidence-based practices from previous years while focusing on a more interactive, group-centered approach. Participants will work together in groups based on their selected quality measure, applying evidence-based strategies and working through each stage of the Performance Improvement Process (PIP) with guidance and support. This collaborative will foster a peer-driven learning environment, allowing participants to share insights, refine their approaches, and track progress. Facility-specific reporting will be provided, ensuring providers can make data-driven decisions while staying informed on updates to Ohio Medicaid reimbursement and QIP changes.

This session will focus on leadership strategies for sustaining momentum in the PIP process and ensuring continuous improvement, even after initial successes. Participants will learn how to lead teams through the ongoing PIP cycle for their selected quality measure, ensuring long-term success and sustained engagement. Leadership will play a crucial role in reinforcing accountability, celebrating progress, and ensuring that improvements remain a priority across the organization. Participants will leave with a plan for how to maintain leadership support and collaboration, ensuring continuous quality improvements in their facility’s operations.


Education

1.0 clock hours will be awarded upon successful completion. 

1.2 CPE hours will be awarded upon successful completion.

Criteria for successful completion: Attendance at entire event and submission of completed evaluation form.

LeadingAge Ohio is an approved provider by the Board of Executives of Long Term Services and Supports. Core of knowledge subject areas: Care, Services and Supports; Operations; Environment and Quality; Leadership and Strategy.

As stated in the Ohio Administrative Code Rule 4723-14-05, education that has been approved by a board or an agency that regulates a health care profession in Ohio or another jurisdiction is an acceptable option to meet nursing continuing education requirements.
