2026 STAT: Survey Tips and Tactics – Injury of Unknown Origin and Abuse Investigation Requirements

Injuries of unknown origin are being identified more frequently during nursing home surveys and complaint investigations and are often cited under the abuse and neglect regulations (F600–F610). CMS expects facilities to promptly identify, investigate, and report injuries when the source of the injury cannot be explained or when there is suspicion of abuse, neglect, or mistreatment. Surveyors commonly cite facilities when staff fail to recognize potential injuries of unknown origin, delay required reporting, conduct incomplete investigations, or fail to document findings and corrective actions. 

This session will review CMS expectations for identifying and responding to injuries of unknown origin, including required reporting timeframes, investigation processes, and documentation standards. Participants will examine common survey findings related to abuse investigations and learn practical strategies to strengthen internal reporting systems, ensure timely investigations, and support staff in recognizing and responding appropriately to potential abuse situations.

Continuing Education

1.0 clock hour will be awarded upon successful completion.

Criteria for Successful Completion: Participant must listen to the entire recorded webinar and follow and complete all steps within the program to receive the CE certificate. This includes the post-test and evaluation.  On the post-test, to qualify for the clock hour, a minimum score of 80% must be achieved.
