This 3-hour in-person refresher training will prepare hospice team members—particularly clinicians and administrators—for the October 1, 2025 go-live of the CMS-mandated Hospice Outcomes & Patient Evaluation (HOPE) Tool. This session, titled HOPE is HERE, will provide updated guidance, reinforce best practices, and address real-world implementation challenges using case-based learning and interactive exercises.

Substitution and Cancellation Policy: Substitutions, in writing, are welcome at not cost. Cancellations received by two calendar weeks prior to event date will be fully refunded. Cancellations received between 7 and 13 days prior to the event date will be refunded minus a $40 processing fee. No refunds will be issued 6 or fewer days prior to the event date.
