In 2025, Resident Records (F842) ranked as the 10th most frequently cited deficiency in complaint surveys nationally and is the 10th most cited for Ohio in 2026, reflecting ongoing challenges with documentation accuracy and record completeness. Surveyors frequently cite F842 when medical records do not accurately reflect the resident’s condition, care provided, services delivered, or significant events that occurred.
This session will review CMS expectations under F842 for maintaining accurate, complete, and reliable medical records, highlight common complaint survey findings related to incomplete or inconsistent documentation, and discuss practical strategies facilities can implement to strengthen documentation practices across departments. Participants will learn how surveyors evaluate medical records during complaint investigations and how improving documentation processes can reduce survey risk while supporting clear communication and continuity of care.
