The updated CMS guidance reinforces the role of QAPI in identifying and addressing health equity concerns within long-term care facilities. Facilities are now expected to incorporate feedback mechanisms to assess whether they effectively meet the needs of diverse populations, including individuals with disabilities, limited English proficiency, and varying cultural or ethnic preferences. Additionally, data collection and monitoring must extend beyond general facility outcomes to evaluate sub-population trends, ensuring that disparities related to race, sexual orientation, socioeconomic status, or preferred language are recognized and addressed. This session will examine the updated surveyor guidance for QAPI programs and their expanded focus on health equity.

The Summit is a comprehensive, one-day conference designed to advance the understanding, accessibility, and integration of palliative care across Ohio. This conference will bring together healthcare providers, health plans, policymakers, and community leaders to discuss strategies for overcoming barriers, improving access, and ensuring quality palliative care for all Ohioans. The event will feature keynotes, moderated panels, and interactive discussions focused on critical aspects of palliative care, including clinical practice, financial viability, workforce development, and innovative models of care. 

Lunchtime will provide an opportunity for attendees to engage in tabletop discussions on pre-identified topics such as financial viability, early access to care, and developing community-based palliative care. The afternoon will focus on increasing access to palliative care, with sessions dedicated to defining patient needs, addressing clinical staffing challenges, and exploring financing models, including reimbursement strategies and medication access. 

In addition, we will discuss the crucial role of advanced care planning and the integration of palliative care into primary care settings. The conference will conclude with closing remarks highlighting the importance of continued collaboration and actions to strengthen and expand Ohio’s palliative care infrastructure for providers and patients. policy in Ohio.

Ensuring the accuracy of resident assessments is a growing priority in the revised State Operations Manual (SOM) Appendix PP, with strengthened surveyor guidance for evaluating compliance. Surveyors are instructed to identify patterns of inaccurate coding that may impact payment, care planning, or quality reporting, with citations issued at a minimum of pattern or widespread scope when noncompliance is found. Facilities found to have systemic inaccuracies—such as MDS assessments or clinical documentation practices that inflate PDPM scores, suppress Quality Measures (QMs), or untrigger Care Area Assessments (CAAs)—may face serious regulatory consequences, including referrals to the State Board of Nursing and the Office of the Inspector General. This session will examine the updated surveyor expectations, discuss common pitfalls in resident assessment accuracy, and provide strategies to ensure compliance while maintaining integrity in reporting and reimbursement.

Palmetto GBA is pleased to announce our 2025 Home Health Workshop series titled “Thrive with Medicare in 2025.” These workshops are designed to equip home health providers with the tools they need to be successful with Medicare billing, coverage and documentation requirements.

The “Thrive” workshops will provide insight for home health agency staff at all levels. The Long-Term Care Survey Process is undergoing significant updates to enhance oversight of staffing compliance across all departments, including nursing, activities, and dietary services. Surveyors are now required to assess staffing sufficiency whenever concerns arise related to quality of care, quality of life, actual harm, or immediate jeopardy. Additionally, new guidance incorporates the PBJ Staffing Data Report as a key investigative tool to identify potential noncompliance. This session will provide an in-depth review of the updated surveyor guidance, investigative procedures, and key triggers for staffing-related citations. Participants will gain insight into how surveyors use PBJ data to assess compliance and learn strategies to proactively address staffing challenges before they become survey deficiencies.

February 27: Survey Strategy and Risk Evaluation 

Learn how to use your facility specific Survey Strategy Worksheet to identify and address risks. This session will cover how to evaluate trends, understand the resident interview process, and incorporate these strategies into daily operations for sustained compliance

March 27:  LTCSP Mandatory Task Observations 

Dive into the LTCSP mandatory tasks, focusing on observation processes across departments. Learn how to conduct effective observations and use findings to enhance compliance and quality improvement

April 24: LTCSP Record Review Process 

Gain a comprehensive understanding of the LTCSP record review process. This session will help participants integrate record reviews into daily compliance activities to sustain survey readiness. Understand the Interdisciplinary Team (IDT) role in mastering the LTCSP record review process to strengthen compliance. May 22:  Managing the Survey Process 

Prepare your team to manage the survey process confidently. This session will provide best practices for planning, assigning roles, and utilizing real-time tools to ensure compliance during surveys

June 26: Post-Survey Process and Sustainability 

Wrap up the collaborative by mastering the post-survey process, including development of a Plan of Correction (POC). Learn how to incorporate POC processes into year-round compliance efforts and understand the implications of Civil Money Penalties (CMPs)
