Participants will learn strategies to ensure residents receive appropriate bowel and bladder management that enhances quality of life, reduces infection risks, and meets regulatory requirements.

    December 10, 2025

Discover how nursing homes are being cited with their annual surveys. The Long-Term Care Survey Process and regulatory language will be reviewed. Bowel and bladder care planning and continence management are often overlooked until surveyors identify deficiencies under F690 (Bowel/Bladder Incontinence, Catheter Care) or related tags. Noncompliance frequently stems from inadequate assessments, lack of individualized toileting programs, or failure to prevent unnecessary catheter use. This session will explore CMS expectations for continence care, restorative nursing programs, and documentation to support interventions. Participants will learn strategies to ensure residents receive appropriate bowel and bladder management that enhances quality of life, reduces infection risks, and meets regulatory requirements.

Elopement incidents remain a high-risk area for survey citations under F689 (Free of Accident Hazards/Supervision/Devices) and often result in Immediate Jeopardy findings when residents leave the facility unsupervised. Surveyors closely examine whether facilities have comprehensive elopement risk assessments, individualized care plans, and effective monitoring systems in place. In 2025, Ohio’s revised nursing home rules expanded and clarified reporting requirements for elopement incidents, requiring facilities to report certain events to the Ohio Department of Health even when they do not meet the federal definition of neglect. This session will review both CMS expectations and Ohio’s updated reporting rules, including preventive interventions, staff training, and post-incident investigations. Participants will analyze real-world examples of elopement-related citations and learn how to strengthen their facility’s protocols to protect residents, maintain compliance, and meet both federal and state requirements.

Immunization compliance remains a critical focus during annual surveys, with F883 (Influenza and Pneumococcal Immunizations) frequently cited when facilities fail to follow regulatory requirements for offering, documenting, and administering vaccines. In 2025, multiple CMS QSO memos and CDC updates have revised recommendations for timing, eligibility, and administration of certain vaccines, including updated guidance on COVID-19, influenza, pneumococcal, and RSV vaccines for older adults. Surveyors will expect facilities to incorporate these updates into policies, education, and recordkeeping. Enhanced Barrier Precautions (EBP) have become a critical component of infection prevention and control efforts in long-term care settings, directly impacting compliance with F880. As F880 remains a top-cited F-tag, failures to implement or maintain EBP protocols are frequently cited during surveys. This session will examine the CMS expectations for EBP implementation, highlight the difference between Standard Precautions and EBP, and explore how surveyors assess compliance with these critical guidelines. Participants will gain practical insights into how to integrate EBP seamlessly into their daily infection control practices, protect residents from multidrug-resistant organisms (MDROs), and ensure their facility’s policies and practices align with CMS requirements.
